Dr. Kleman is paneled with Medicare only; she is not in-network for other insurance carriers. 

Medicare Patients

Medicare patients are asked to provide a credit card at the time of scheduling and will be charged a no-show fee of $500 if they no-show, cancel, or reschedule with less than 48 hours notice. Dr. Kleman will bill Medicare on their behalf for their evaluation.

All Other Patients

All non-Medicare patients will be charged a flat rate depending on the purpose and length of the evaluation. They will be charged a deposit when scheduling their first appointment that serves as a no-show fee in the event that they no-show, cancel, or reschedule their appointment with less than 48 hours notice. The deposit is applied to their evaluation fee and the final balance is due at the first appointment. After completion of the evaluation, Dr. Kleman will bill your insurance as an out-of-network provider and will refund the amount that your insurance company reimburses.
