FINANCIAL INFORMATION

Dr. Kleman is paneled with Medicare only; she is not in-network for other insurance carriers. All patients are asked to provide a credit card upon scheduling an appointment and will be charged a $500 fee if they no-show or cancel with less than 48 hours notice. 

Medicare Patients

Dr. Kleman will bill Medicare on their behalf for their evaluation. In the event that a balance is due after insurance, patients will be sent an invoice. 

All Other Patients

All non-Medicare patients will be charged a flat rate depending on the purpose and length of the evaluation. After completion of the evaluation, Dr. Kleman will bill your insurance as an out-of-network provider and will refund the amount that your insurance company reimburses. Alternatively, if you prefer to submit a super bill to your insurance company, we are happy to provide the documentation needed.

We accept debit cards, credit cards, checks, and cash.
